Just enter your URLs into the “$eur_urls”-array like this:
$eur_urls = array(
“my-cool-url” => “my/absolute/path/to/my/cool/file.php”
);

Copy the plugin to your plugins directory.
Activate the plugin.
Activate permalinks.
